In most instances, we can see you within a few days, and on some occasions the same day.

New Patients: Thank you for giving us the opportunity to serve you!

As a courtesy to you and the trees, we try and send all documentation electronically. Please contact our office so we may send you the appropriate forms. If you prefer to complete the forms in office, please arrive 15 minutes before your scheduled appointment to allow us proper time for registration.

 

Please bring us your dental and medical insurance card(s), and a list of all medications you are currently taking. Please make sure you take all your usual medication on the day of the first visit, and antibiotics if your medical doctor requires that you take them prior to dental appointments.

 

Patients 18 years of age and younger must be accompanied by a parent or legal guardian.

 

As a courtesy to other patients and our team, please call the office as soon as possible if you are unable to keep your appointment or are going to be late.
